Pork stored in a Glad quick-tie bag, a live roach on "wooden chicken-pounding prep table," an employee returning from the bathroom and then removing food from the reach-in cooler without washing their hands and more than 100 dead roaches inside a walk-in cooler with food were among issues that temporarily closed a dozen restaurants in South Florida last week.
